Case Study: A Prosthodontist Restores A Damaged Smile

As a prosthodontist, my primary goal is to restore teeth using prosthetics, focusing on achieving both functional and aesthetically pleasing results. In my daily practice in downtown Los Angeles, I work on a range of cases that require the precise design and implementation of dental restorations. Today, I invite you to join me in exploring a recent case that exemplifies the core of my profession.

Prosthodontics is the art and science of rebuilding smiles, and my role often involves collaborating with other dental specialists to provide comprehensive care to our patients. This case offers a glimpse into prosthodontics, shedding light on the steps, techniques, and considerations that go into restoring a patient’s dental health and confidence.

In the following case study, I’ll walk you through this typical case, highlighting the intricacies of my work and, ultimately, demonstrating how prosthodontics can make a significant difference in the lives of those seeking to regain their dental function and radiant smile.

The Problem

The patient’s initial presentation was relatively common in prosthodontic practice, but it highlighted a significant issue. They sought my expertise with a concern: two chipped porcelain veneers on their upper front teeth. However, upon closer examination, it became apparent that the problem extended beyond the veneers. See figure 1.

The gum tissue adjacent to these veneers appeared red and inflamed, signaling potential underlying issues. To get a comprehensive view of the situation, I decided to remove the veneers and perform a thorough examination. What I discovered was contamination within the root canals of the affected teeth.

The root canals, situated in the innermost part of a tooth, contain the nerve and pulp. In this case the canals had already been previously filled, but were contaminated with bacteria. It is critical that the canals are clean and sealed before proceeding to the final restoration.

The Solution

Before we could restore the patient’s teeth with new restorations, a crucial step was to collaborate with a respected endodontist who also practices in downtown Los Angeles.

An endodontist is a dentist who specializes in treating problems related to tooth pulp. In this case, their expertise was invaluable. The endodontist performed a meticulous retreatment of the patient’s root canals. This procedure involved removing the old root canal fillings, disinfecting and sealing the canals with a new biocompatible material. This helps to prevent future infections and ensures a healthy foundation for the new restorations. 

After the canals were retreated, I then carefully created the necessary space within the canals to accommodate new custom posts. These posts are casted by my laboratory technician in gold alloy. Figure 3 shows the X-rays during the treatment. The X-ray on the left shows the pre-treatment image of the root canals and the chipped veneers. The middle X-ray shows the re-treatment of the canals with space created for the posts. The X-ray on the right shows the gold posts cemented into the canals.

Figures 4 shows the red acrylic pattern made for the posts. Figure 5 shows the custom gold posts in position after casting by the laboratory. These posts are retained by the canals and are critical for stabilizing the new crowns. 

This cooperation between myself as the prosthodontist, the endodontist and the laboratory technician illustrates a fundamental aspect of our roles. As prosthodontists, we often serve as the “quarterback” of a patient’s dental team. We chart the course of treatment, coordinating procedures with specialists to optimize the patient’s oral health and deliver the best possible outcome.

The End Result

The culmination of this comprehensive treatment was nothing short of stunning, as depicted in these final photographs. The transformation is not only remarkable but also profoundly gratifying for both the patient and our dental team.

One key aspect contributing to the aesthetic success of the treatment is the meticulous design of the crowns. See figure 6 below. They were crafted with precision, with particular attention given to blocking the gold color of the underlying support posts. This attention to detail ensured the teeth did not appear unnaturally dark or discolored. Instead, the crowns seamlessly blend with the surrounding natural teeth, achieving a harmonious and visually pleasing result.

The porcelain layer meticulously applied over the zirconia crowns plays a pivotal role in mimicking the light-reflecting qualities of natural tooth enamel. This characteristic is crucial in achieving a result that not only looks natural but also exudes the radiance and vitality of a genuine smile. See figures 7 and 8 below.

With this successful treatment, the patient has regained more than just their dental function; they have reclaimed their confidence and a beautiful smile. The ability to comfortably enjoy everyday activities, from speaking and chewing to sharing a confident smile, is a testament to the transformative power of prosthodontics.

The Patient’s Concern A patient was referred to Geach Dental because she was unhappy with the appearance of her smile. She disliked the spaces between her front teeth, the uneven coloration, and the white spots that distracted from an otherwise healthy smile. Like many cosmetic dentistry patients, her goal was not to have an artificial “Hollywood smile.” She wanted a smile that looked naturally beautiful, balanced, and harmonious with her face. The Hidden Problems Behind the Smile When designing a smile makeover, it is important to look beyond the teeth alone. During our comprehensive smile analysis, we identified several factors affecting the appearance of her smile: Many cosmetic dental treatments focus only on the teeth. However, truly exceptional smile makeovers require careful evaluation of both the teeth and the surrounding gums. Designing the Ideal Smile Every smile makeover at Geach Dental begins with detailed planning. Using digital smile design technology, we created a customized preview of the patient’s future smile. This allowed us to evaluate tooth shape, proportions, symmetry, and overall facial harmony before treatment even began. One of the most important findings was that the length of the patient’s front teeth was already ideal for her facial features. Making the teeth longer would have created an unnatural appearance. Instead, the goal was to improve symmetry, close the spaces, and create a more youthful and balanced smile. Creating Better Gum Symmetry The gums frame the teeth just as a picture frame surrounds artwork. Because the patient’s gumline was uneven, we coordinated treatment with a periodontal specialist who performed conservative gum recontouring. This minor procedure improved symmetry and created more ideal tooth proportions. By correcting the gum architecture first, we established the foundation for a beautiful and natural-looking final result. Why Crowns Were Chosen Instead of Veneers One of the most common questions patients ask is whether veneers or crowns are the better option. In this case, custom all-ceramic crowns provided superior control over tooth shape and contour. This allowed us to close the spaces between the teeth while maintaining natural proportions and creating a seamless appearance. The treatment involved six upper front teeth, carefully designed to complement the patient’s facial features and smile. Precision Smile Design and Shade Matching Creating a natural smile requires more than simply choosing a white shade. We worked closely with our dental laboratory to select the ideal color, translucency, and surface characteristics for the patient. Every detail was customized to ensure the restorations blended naturally and reflected light like healthy natural teeth. Temporary restorations were created first so the patient could preview the new smile before the final restorations were fabricated. The Final Result The final restorations were crafted from high-quality zirconia ceramic, a material known for its strength, durability, and lifelike appearance. The completed smile makeover achieved: Most importantly, the patient achieved the confidence that comes from smiling freely without feeling self-conscious about her teeth. Full-Arch Dental Implant Rehabilitation Case Report

Restoring Function, Comfort, and Confidence with Implant Dentistry in Los Angeles & Orange County When patients lose multiple teeth or experience severe dental breakdown, everyday activities like chewing, speaking, and smiling can become difficult and frustrating. In this complex implant reconstruction case, Adam Geach, DMD, MDSc — Prosthodontist at Geach Dental — restored a patient’s bite, function, and appearance using advanced dental implant treatment and full-mouth reconstruction techniques. This type of case highlights why specialized prosthodontic training matters when rebuilding a patient’s occlusion with dental implants. The Patient’s Concerns The patient came to Geach Dental with several serious dental problems, including: These issues not only affected appearance but also severely impacted daily function and long-term oral health. For many patients in Los Angeles and Orange County, this situation is more common than they realize. Missing back teeth and failing dental work can gradually cause the bite to collapse over time, leading to difficulty eating, jaw strain, and accelerated tooth wear. See pictures below.  Diagnosis: Advanced Bite Collapse and Tooth Loss After a comprehensive prosthodontic evaluation, the diagnosis included: Because the remaining lower teeth could not be predictably saved, a full-arch implant reconstruction was recommended. Advanced Digital Treatment Planning Complex implant cases require careful planning before any treatment begins. At Geach Dental, advanced digital scanning and prosthodontic treatment planning were used to determine the ideal tooth position, bite relationship, smile design, and implant placement. One of the most important goals in implant dentistry is not simply replacing teeth — it is restoring the bite in a healthy, stable, and natural way. Most of the patient’s upper teeth were healthy enough to preserve, while the remaining lower teeth required extraction due to extensive bone loss. Full-Arch Dental Implant Reconstruction After removing the failing lower teeth, precision-guided implant surgery was performed. Treatment Included: Following a healing period of approximately three months, the implants were restored with: Unlike removable dentures, this implant bridge is permanently fixed in place, giving the patient a much more natural and secure experience while eating and speaking. A Smile Designed for Function and Long-Term Health At Geach Dental, implant restorations are designed not only for esthetics, but also for long-term maintainability and hygiene. The lower implant bridge was carefully designed to allow the patient to floss effectively and clean around the implants using specialized hygiene tools such as a water flosser. The Final Result The completed reconstruction restored: The patient can now comfortably eat a wide range of foods without worrying about loose dentures or unstable teeth. An additional benefit of this case was the ability to preserve the patient’s healthy upper front teeth. Minor cosmetic recontouring was performed to improve the appearance while maintaining natural tooth structure.
