Full-Arch Dental Implant Rehabilitation Case Report

Restoring Function, Comfort, and Confidence with Implant Dentistry in Los Angeles & Orange County

When patients lose multiple teeth or experience severe dental breakdown, everyday activities like chewing, speaking, and smiling can become difficult and frustrating. In this complex implant reconstruction case, Adam Geach, DMD, MDSc — Prosthodontist at Geach Dental — restored a patient’s bite, function, and appearance using advanced dental implant treatment and full-mouth reconstruction techniques.

This type of case highlights why specialized prosthodontic training matters when rebuilding a patient’s occlusion with dental implants.

The Patient’s Concerns

The patient came to Geach Dental with several serious dental problems, including:

  • Multiple missing teeth
  • Loose and failing lower teeth
  • Advanced periodontal bone loss
  • Difficulty chewing comfortably
  • Shifting teeth and collapsing bite
  • An aging, worn appearance caused by tooth loss and bite collapse

These issues not only affected appearance but also severely impacted daily function and long-term oral health.

For many patients in Los Angeles and Orange County, this situation is more common than they realize. Missing back teeth and failing dental work can gradually cause the bite to collapse over time, leading to difficulty eating, jaw strain, and accelerated tooth wear. See pictures below.

Diagnosis: Advanced Bite Collapse and Tooth Loss

After a comprehensive prosthodontic evaluation, the diagnosis included:

  • Missing posterior (back) teeth
  • Severe periodontal disease in the lower jaw
  • Significant bone loss around lower teeth
  • Tooth shifting and migration
  • An overclosed bite
  • Reduced chewing efficiency and function

Because the remaining lower teeth could not be predictably saved, a full-arch implant reconstruction was recommended.

Advanced Digital Treatment Planning

Complex implant cases require careful planning before any treatment begins. At Geach Dental, advanced digital scanning and prosthodontic treatment planning were used to determine the ideal tooth position, bite relationship, smile design, and implant placement.

One of the most important goals in implant dentistry is not simply replacing teeth — it is restoring the bite in a healthy, stable, and natural way.

Most of the patient’s upper teeth were healthy enough to preserve, while the remaining lower teeth required extraction due to extensive bone loss.

Full-Arch Dental Implant Reconstruction

After removing the failing lower teeth, precision-guided implant surgery was performed.

Treatment Included:

  • 2 upper dental implants to replace missing upper molars
  • 5 lower dental implants to support a full-arch fixed implant bridge
  • Digitally guided implant placement for precision and long-term stability

Following a healing period of approximately three months, the implants were restored with:

  • Individual implant crowns in the upper jaw
  • A custom full-arch fixed bridge in the lower jaw

Unlike removable dentures, this implant bridge is permanently fixed in place, giving the patient a much more natural and secure experience while eating and speaking.

A Smile Designed for Function and Long-Term Health

At Geach Dental, implant restorations are designed not only for esthetics, but also for long-term maintainability and hygiene.

The lower implant bridge was carefully designed to allow the patient to floss effectively and clean around the implants using specialized hygiene tools such as a water flosser.

This is a critical aspect of successful implant dentistry that is often overlooked.

The Final Result

The completed reconstruction restored:

  • Proper bite function
  • Improved chewing efficiency
  • Smile esthetics
  • Facial support
  • Long-term stability and comfort

The patient can now comfortably eat a wide range of foods without worrying about loose dentures or unstable teeth.

An additional benefit of this case was the ability to preserve the patient’s healthy upper front teeth. Minor cosmetic recontouring was performed to improve the appearance while maintaining natural tooth structure. See pictures below of the final result.

Why Choose a Prosthodontist for Dental Implants?

A prosthodontist is a dental specialist with advanced training in:

  • Dental implants
  • Full-mouth reconstruction
  • Complex bite rehabilitation
  • Cosmetic dentistry
  • Smile design
  • Restoring damaged or failing dentition

Cases involving severe tooth loss, collapsing bites, or advanced implant reconstruction require far more than simply placing implants. Proper bite design, esthetics, jaw function, and long-term durability all need to work together.

At Geach Dental, patients receive highly customized implant and reconstructive treatment designed specifically for long-term health, function, and esthetics. If you have missing teeth and are looking to improve your chewing function and esthetics, we encourage you to contact Geach Dental at (213) 622-3339, or schedule a consultation. You deserve the best care.

Replacing Old Crowns, Veneers and Dental Implant Restorations for a Natural, Younger Smile

When Old Dental Work Makes Your Smile Look Older Even the highest-quality dental work doesn’t last forever. Over time, crowns, veneers and implant restorations can begin to show their age. They may chip, stain, or no longer match your surrounding teeth. As your gums naturally change over the years, restorations that once looked beautiful can begin to appear uneven or unnatural. Many patients tell us: This patient came to Geach Dental looking for exactly that. The Challenge This patient had several cosmetic concerns affecting the upper front teeth: Because the implant could not simply be moved, restoring this smile required careful planning and advanced prosthodontic expertise rather than simply replacing the crowns.  Why Replacing Old Dental Work Isn’t Always Simple Many people assume replacing an old crown or veneer is straightforward. Unfortunately, that’s not always the case. When dental implants, veneers, crowns and natural teeth all need to work together, every small detail matters: These complex cases are exactly why prosthodontists receive years of additional specialty training after dental school. Our Treatment Process Rather than immediately making new crowns, we first removed the existing restorations to evaluate the health of the underlying teeth and assess the position of the dental implant. Using digital smile design principles, we created custom provisional (temporary) restorations that allowed us to: These temporary restorations became the blueprint for the final result. Precision Craftsmanship Makes the Difference One advantage unique to Geach Dental is our in-office dental laboratory. Instead of sending everything to an outside lab, Dr. Adam Geach works directly with an experienced laboratory ceramist to customize: This close collaboration allows us to achieve an exceptional level of detail that is especially important in the highly visible front teeth. The Final Result Although the original implant position limited how much the gum line could be changed, nearly every other esthetic concern was corrected. The new restorations provided: The result is a smile that looks healthier, more balanced and significantly more natural. Healthy implant and tooth structure formed first: Final Restorations Placed: Do You Have Old Crowns, Veneers or Dental Implant Restorations? You may be an excellent candidate for replacing your existing dental work if you notice: Many patients are surprised how dramatically replacing aging dental work can refresh their entire appearance. Why Choose a Prosthodontist? Replacing cosmetic dental work is about much more than making new crowns. A prosthodontist is a dental specialist trained in complex restorative, cosmetic and implant dentistry. This advanced training is especially valuable when treating challenging cases involving: At Geach Dental, every smile is carefully planned with the goal of creating results that are beautiful, functional and built to last. When a Dental Implant Becomes a Challenge: How We Restored an Extremely Difficult Implant Case

Not Every Dental Implant Can Be Restored the Same Way Most dental implants are straightforward. A single implant is placed where a missing tooth once was, heals for several months, and then receives a custom crown that looks and functions like a natural tooth. See the example below.   Occasionally, however, patients are referred to our office with implant situations that are far more complicated. This was one of those cases. The Problem: Two Implants Where Normally Only One Is Needed This patient was referred to our prosthodontic practice after two very narrow dental implants had been placed in the space of a single lower molar. See picture below.  Although the implants were healthy, they were positioned extremely close together and angled toward each other. This unusual placement made it nearly impossible to place a conventional implant crown. For many dentists, this type of case can become frustrating because standard implant components simply won’t fit. Instead of replacing the implants, we developed a customized solution designed specifically for this patient. Why This Was So Difficult Think of trying to place a lid onto two jars that are leaning toward each other. If the jars aren’t parallel, the lid won’t fit. The same concept applies to dental implants. Because these implants were angled differently, a traditional implant crown could not seat properly. Without advanced prosthodontic planning, the patient may have required extensive additional treatment—or even implant replacement. Our Solution: Custom Implant Engineering Rather than removing healthy implants, we designed two custom implant abutments. These precision-made components corrected the angulation of the implants and created a common path that allowed a single implant crown to fit accurately. See the X-ray below showing the tapered, custom abutments. Everything was designed, and fabricated with exceptional precision in our in-office dental laboratory. This approach allowed us to preserve the existing implants while creating a restoration that is both functional and attractive. The Final Result The completed implant crown: Most importantly, the patient left with a tooth that functions naturally without requiring additional surgery.
